    The short answer

    Record the name as printed, the dose and form, the time you actually took it, why you take it, and start and stop dates — for prescriptions, over-the-counter medicines and supplements alike. Note how you felt afterwards as an observation with a time, never as proof of cause, and ask a pharmacist about interactions.

    Why supplements belong in the same list

    People often list prescriptions when asked what they take, and leave out vitamins, herbal products and over-the-counter medicines. Regulators are explicit that dietary supplements can interact with medicines and are not reviewed for safety and effectiveness the way prescription drugs are. Your clinician and pharmacist can only account for what they know about.

    What a complete entry looks like

    • Name as printed on the package, including the brand where relevant.
    • Dose and form — 400 mg, one tablet, 5 ml.
    • When you took it, not when you were supposed to.
    • Why you take it, in one line.
    • Started and stopped dates. Stopping is as informative as starting.
    • Anything you noticed afterwards, recorded as an observation with a time.

    "How I felt after taking it" — and what it means

    Noting how you felt after taking something is genuinely useful information for a conversation. It is not evidence of cause. Symptoms fluctuate on their own, several things change at once, and expectation shapes experience. Write the observation and the time, and let a clinician weigh it.

    Keep one list, not four

    A single current list — prescriptions, over-the-counter medicines, supplements, and anything taken occasionally — is what a pharmacist, a new clinician or an emergency department actually needs. Update it when something changes rather than rebuilding it from memory each time.

    What tracking does not replace

    • It does not check for interactions. Ask a pharmacist, who can check your full list.
    • It does not tell you whether to start, stop or change a dose. Never change prescribed medication on the basis of your own log alone.
    • It is not a diagnosis and not a medical record.
